Participants are placed in an operating room where they are given the difficult task of treating a patient.

The gameplay of Surgeon Simulator 2 is completely based on real laws of physics. All patients are called Bobs and their lives are at the mercy of the players. Once at the Bossa Labs hospital, you must use a wide variety of surgical instruments to perform a critical operation. Patients can have any body parts cut off, their entrails taken out, and much more.

You will need to choose a task: head transplant, leg amputation and much more. There are relay races and dance competitions. Story mode is also available. It can be played alone or with friends. Various goals are set here: opening chests, organ transplants, regulating the amount of blood and much more. Sometimes hospitals can be a tangle of corridors and traps.

Operations are carried out in a classic hospital, on a space station, in a disco hall, in a bowling club, at a carnival and in other extreme places. The sequel significantly expands the gameplay mechanics and introduces a lot of different content. Creation Mode is also available, where you can create your own levels.
