The game includes three new routes: the West Coast Main Line from London Euston to Milton Keynes, the Frankfurt - Fulda route in Germany and the San Bernardino Line in the US. Each route offers gamers unique challenges and opportunities to explore different types of train controls. One of the features of Train Sim World 5 is Conductor Mode, which allows users to take on the role of a train conductor, managing the boarding of passengers and ensuring timely departures.

This mode adds a new level of realism and responsibility, making the gameplay more immersive. In addition, the project introduced a Route Hopping mode, allowing players to seamlessly switch between different routes, making virtual railway travel more continuous. The title also includes many new locomotives and trains, such as the Metrolink MP36, Class 350/1 and Class 377/2 EMU, as well as the DB BR 411 ICE-T. These add-ons provide gamers with a wide variety of trains to choose from, each with their own set of controls and features.
